Peer Support Services · The Triad and surrounding counties
Recovery is easier with someone who has been there.
4ward Steps pairs adults on NC Medicaid with certified peer support specialists who have their own lived experience of recovery from mental health or substance use challenges.
What peer support is
A partner in your recovery, not another appointment to sit through
A peer support specialist is a North Carolina Certified Peer Support Specialist who has lived through their own recovery from a mental health or substance use challenge. They use that experience to walk alongside you while you work toward the life you want.
Your specialist meets you where you are, at home or out in your community, and helps you work on the goals in your plan. Peer support works side by side with your clinical care. It does not replace your counselor, doctor or medications, and it is not a crisis service.

Who we serve
Adults working toward recovery
Peer support at 4ward Steps is for adults 18 and older who:
- live with a serious mental illness or a substance use disorder
- want a peer to support their recovery
- have NC Medicaid through one of the health plans listed here
We match you with a specialist whose own experience is similar to yours. If peer support isn't the right fit, we'll help you find a service that is.
Health plans we work with
For referring providers: Peer Support Services are billed to NC Medicaid as H0038 and need a comprehensive clinical assessment and prior authorization from the person's health plan.

Effective date: November 4, 2026
Our duties
4ward Steps LLC provides peer support services. The law requires us to keep your health information private, to give you this notice of our legal duties and privacy practices, to follow the notice now in effect, and to tell you if a breach puts your unsecured information at risk. We follow federal privacy law (HIPAA), the federal rules for substance use disorder records (42 CFR Part 2), and North Carolina law (G.S. 122C). When one law gives you more protection, we follow that law.
How we may use and share your information without your written permission
- Treatment: to provide and coordinate your peer support services, for example talking with the clinician who completed your assessment or with your care manager.
- Payment: to bill and be paid by your health plan, for example sending your service dates and authorization requests to NC Medicaid or your plan.
- Health care operations: to run our program, for example reviewing records for quality, training staff and preparing for accreditation surveys.
North Carolina law is stricter than federal law for mental health and substance use services, so in many cases we will ask for your written consent even for these purposes.
Other times the law allows or requires us to share information
- When required by federal, state or local law.
- To report suspected abuse, neglect or exploitation of a child or a disabled adult.
- To prevent a serious and imminent threat to your health or safety or that of others, for example calling 911 or a crisis team.
- For health oversight, such as audits and licensing or accreditation reviews.
- For public health activities, when the law requires.
- In response to a court order. Substance use disorder records are released for a court case only with your written consent or a court order that meets federal Part 2 rules.
- To medical examiners or coroners, and for certain specialized government functions, when the law requires.
When we need your written permission
We will not share your information for any other reason, including marketing, without your written authorization. We do not sell health information and we do not fundraise. You may cancel (revoke) an authorization in writing at any time, except for information already shared because of it.
Substance use disorder records
- Records about substance use disorder diagnosis or treatment have extra federal protection under 42 CFR Part 2.
- You may give one written consent for us to use and share these records for treatment, payment and health care operations, and you may revoke it in writing at any time.
- Records shared with your consent may not be shared again by the person who receives them, except as federal law allows.
- These records, or testimony about them, cannot be used in a civil, criminal, administrative or legislative case against you unless you give written consent or a court order meets federal requirements.
Your rights
- See or get a copy of your records. Ask in writing. We will respond within 30 days and may charge a reasonable, cost-based fee for copies.
- Ask us to correct information you believe is wrong or incomplete. Ask in writing. If we say no, we will tell you why in writing.
- Ask us to limit what we use or share. We do not have to agree, except that we must agree not to share with your health plan information about a service you paid for in full yourself.
- Ask us to contact you in a certain way or at a certain place, such as a different phone number. We will agree to reasonable requests.
- Get a list (accounting) of certain times we shared your information in the past 6 years, other than for treatment, payment, health care operations or with your permission. One list a year is free.
- Get a paper copy of this notice at any time, even if you received it electronically.
- Choose someone to act for you, such as a legal guardian or health care power of attorney.
